We describe the process as a journey as it requires long-term commitment to real change which can often come with bumps in the road. The rapid weight loss after bariatric surgery brings hope and a renewed vitality, so when you hit a plateau in your weight loss or even regain weight some years later, this bump in the road can be incredibly disheartening. </span></p>

<p><span style="font-weight: 400;">After <a href="https://ugisb2.dev.purpleeffect.com/bariatric-surgery/" target="_blank" rel="noopener">bariatric surgery</a>, the restrictive <a href="https://ugisb2.dev.purpleeffect.com/what-is-the-post-bariatric-surgery-diet/" target="_blank" rel="noopener">post-surgery diet</a> and changes in appetite and capacity results in rapid weight loss for patients. We expect most patients to lose around 30% or more of their starting weight and have many strategies to help if someone struggles to meet their goals.  After surgery, following a new healthy diet and becoming more active will ensure a steady weight loss. Some people will achieve what they need quickly, others may need more help along the way, but weight generally settles at a much lower and far healthier level than before surgery (if lifestyle changes have been made and committed to). </span></p>

<li style="font-weight: 400;" aria-level="1"><span style="font-weight: 400;">Are you sticking to your post-surgery diet? 3 meals a day? Plenty of protein? </span></li>
<li style="font-weight: 400;" aria-level="1"><span style="font-weight: 400;">What are your activity levels like, could you be doing more or trying something different? </span></li>
<li style="font-weight: 400;" aria-level="1"><span style="font-weight: 400;">Should we be considering the addition of </span><span style="font-weight: 400;">Anti-Obesity medications</span><span style="font-weight: 400;"> such as GLP-1 agonists to help you get to goal?</span></li>

<p><span style="font-weight: 400;">Typically, some weight regain can be expected to occur after 12-24 months, and while in some the weight gain is minor, in others it is progressive which can be very disheartening. From our clinical experience, patients who attend regular follow-ups with their clinic tend to regain less weight than those who don’t come back for follow-ups.</span></p>

<p><span style="font-weight: 400;">While fluctuations are normal, a gradual and persistent weight gain over an extended period may signal the need for attention. We advise our patients to come in and see us if they have gained more than 3kg. If someone has regained &gt; 20% of their lost weight (i.e, if they lose 40 kg and regain more than 8kg) then we would prioritise an appointment to discuss active therapies, as diet and lifestyle may not be sufficient to meet their needs. Furthermore, weight regain beyond this level is likely to create increasing risks of ill health. </span></p></div>

<p><span style="font-weight: 400;">Weight regain seems to occur almost exclusively in people who are not in follow-up. It seems that people who are gaining weight choose not to come in for checks or to receive the support they need, either because they feel guilt about weight regain or because they view weight regain at the time as being unimportant. In reality we want people to return if they experience weight regain, reflux or abdominal pain because this is when our expertise counts the most.</span></p></div>

<p><span style="font-weight: 400;">While weight loss medications can produce positive results during their administration, it&#8217;s important to acknowledge the transient nature of their effects. Many individuals experience initial success only to find that, when they stop taking the medication, the weight creeps back on. The hard truth is that without a fundamental shift in behaviour, relying solely on weight loss medications may not offer a lasting solution. Despite the fact that no-one wants to regain weight after stopping these medications, on average most people will regain &gt;50% of the weight they have lost within a year.</span></p>

<p><span style="font-weight: 400;">There is also nothing ‘temporary’ about a medication if you have to take it long-term to achieve and maintain weight loss. These medications each come with potential side effects and risks that need to be considered by the individual to determine if they are suitable candidates. </span></p></div>

<p><span style="font-weight: 400;">It is also important to note that currently in Australia, there have been supply chain issues with certain medications, meaning fulfilling prescriptions has become difficult for many. This in itself can cause issues because if you do not have access to the prescribed medication, you are at risk of regaining any weight lost. </span></p></div>

When going to a social event choose water first, especially if you’re planning to have a glass of wine. Drinking alcohol when thirsty or tired is never a good idea.</span></p></div>

				<div class="et_pb_text_inner"><p><span style="font-weight: 400;">In response to criticism about previous statements that were directly stigmatising of persons with a lived experience of severe obesity, the Therapeutics Goods Association (TGA) and others have appropriately altered their language regarding the use of GLP-1 agonists which have clinical effectiveness for the management of Type II Diabetes as well as clinically significant obesity. They have now asked clinicians prescribing these medications to consider delaying prescription unless other suitable alternatives are not available, rather than seeking to deny patients with severe obesity the opportunity to receive treatment. </span></p>
<p><span style="font-weight: 400;"></span></p>
<p><span style="font-weight: 400;">While the best of these agents are only approved for use in Australia for the treatment of Type II Diabetes, ample research data and clinical experience are showing excellent results when used for severe obesity, which has subsequently led to widespread “off-label” prescription by Obesity Practitioners in Australia. Some versions of these drugs which are approved as obesity therapies are clinically inferior and more expensive than the newer and potentially scarcer agents. </span></p></div>

				<div class="et_pb_text_inner"><p><span style="font-weight: 400;">The widespread success stories of people living with severe obesity who have lost weight with these medications have subsequently been shared on social media support groups set up by patients, which has then led regulatory agencies and the press to be aware of the potential contribution of these prescriptions to medication shortages. Unfortunately, this has potentially trivialised the importance of providing effective anti-obesity therapy to those in need, and it denies the validity of their experiences and health as being important.</span></p>
<p>&nbsp;</p>
<p><span style="font-weight: 400;">While it is obvious that a poorly available therapy should not be used for frivolous purposes, appropriate consideration should be given to the needs of people living with severe obesity before denying them the opportunity to receive appropriate care. Overweight and obesity have skyrocketed in prevalence over the last 40 years and is Australia’s second largest cause of fatal disease and the largest contributor to non-fatal disease. </span></p>
<p>&nbsp;</p>
<p><span style="font-weight: 400;">While substantial increases in the prescription of diabetic and cardiovascular therapies over this time have blunted the effects of obesity on mortality rates, little has been done with regard to managing obesity itself. No one is going to argue about the importance of lifestyle changes in obesity prevention as part of the suite of interventions for those living with obesity-related diseases. However, we have overwhelming evidence that lifestyle change as sole therapy is universally inadequate for those with severe disease, even though its effects in so-called multi-modality or combined therapies are important. </span></p>
<p>&nbsp;</p>
<p><span style="font-weight: 400;">We know enough about obesity to predict, before treatment, the likely outcomes for an individual depending on the treatment prescribed. Patients requiring health improvement and weight stability should therefore be prescribed differing obesity therapies than those presenting with poorly controlled medical conditions, disabilities or diseases likely to become life-threatening soon. </span></p>
<p>&nbsp;</p>
<p><span style="font-weight: 400;">While offering obesity therapies to patients has been made more difficult by the relative scarcity and complexity of treatments until reasonably recently, those treatments that are suitable, successful and cost-effective are rarely offered. The cost of therapy is often cited as a reason, but there appears to be a double standard applied to those seeking or who are eligible for </span><a href="https://ugisb2.dev.purpleeffect.com/weight-loss-treatment/weight-loss-medication/"><span style="font-weight: 400;">obesity treatment</span></a><span style="font-weight: 400;">. Therapies for obesity that are known to be more cost-effective than what we currently offer other patients with chronic disease (such as renal failure, osteoarthritis etc) or which are even potentially health-cost neutral, are consistently denied to patients by Federal and State healthcare systems. </span></p>
<p>&nbsp;</p>
<p><span style="font-weight: 400;">Clinically severe obesity is the only major health condition where all healthcare costs are expected to be covered by the person with the disease. Most successful medical therapies for obesity are not approved for use by the TGA and are prescribed “off-label”, and even those that are TGA approved receive no government subsidies, which somewhat negates the point of seeking TGA approval in the first place. Patients presenting for treatment in public hospitals with life-threatening or disabling obesity-related conditions will have their condition treated but are virtually guaranteed to receive no obesity directed care other than documentation of their body mass index (BMI). </span></p>
<p>&nbsp;</p>
<p><span style="font-weight: 400;">It is beyond the scope of this opinion piece to discuss the many complexities of obesity stigma, but for those who are interested, </span><a href="https://www.obesityaustralia.org/"><span style="font-weight: 400;">The Obesity Collective</span></a><span style="font-weight: 400;"> provides succinct and easily readable information for the public and lay-press, and the </span><a href="https://auswin.org.au/"><span style="font-weight: 400;">Weight Issues Network</span></a><span style="font-weight: 400;">, a person-centred support group created by those with a lived experience of obesity, for those with a lived experience of obesity, have created an excellent position statement detailing the experiences of seeking health care when one has obesity. It’s unlikely that anyone reading these documents would come away without increasing their understanding and empathy for those negotiating their lives and the healthcare system with obesity as a front and centre issue.</span></p>
<p>&nbsp;</p>
<p><span style="font-weight: 400;">A more nuanced way of managing the potential conflict between the requirements of those with severe diabetes or obesity is to make decisions based on the clinical needs of the patient. There are multiple drugs known to improve diabetes and prolong survival approved for use in Australia. If a diabetic patient does not require significant weight loss and is being prescribed one of the new GLP-1 agonists that are in short supply, alternatives could be prescribed without sacrificing their care. </span></p>
<p>&nbsp;</p>
<p><span style="font-weight: 400;">Similarly, if a patient is being prescribed a GLP-1 agonist for obesity and is at the weight maintenance part of their therapy, other alternatives are available. Those requiring a lesser degree or slower weight loss have other options as well. This would leave scarce agents available for those patients requiring reliable and consistent weight loss to treat significant illness, and for those diabetics where significant weight loss and lifestyle modification are part of the management of their complicated diabetes associated illnesses. </span></p>
<p>&nbsp;</p>
<p><span style="font-weight: 400;">GLP-1 agonists are part of the first of a suite of anti-obesity medications being released which are effective and well-tolerated when appropriately prescribed. Whether or not these make it into the hands of those in need will depend on our Health system&#8217;s re-evaluation of the importance of the needs of those affected by severe obesity, as well as advocacy by patients, clinicians and healthcare institutions.</span></p>

<p><span style="font-weight: 400;">Single Anastomosis Gastric Bypass/Mini Gastric Bypass  (SAGB/MGB), also known as the Omega Loop Gastric Bypass (OAGB), is a more recent innovation in bariatric surgery. Unlike the RYGB procedure, it involves just one connection between the stomach pouch which is always long and narrow, and the small intestine. This approach leads to weight loss using the same mechanisms as the RYGB while simplifying the procedure. Some surgeons lengthen the small bowel bypass &#8211; this may possibly lead to more weight loss but will also increase the chances of persistent diarrhoea and nutrient deficiencies.</span></p></div>

<li style="font-weight: 400;" aria-level="1"><span style="font-weight: 400;">Eat lots of plant foods, i.e. fruits, vegetables, legumes and nuts/seeds. They can be fresh, dried, canned or frozen (just check that they don’t have any added sugar or high salt content). </span></li>
<li style="font-weight: 400;" aria-level="1"><span style="font-weight: 400;">Include seafood in your diet at least twice a week, whilst aiming to reduce meat (red or processed) in your diet. Poultry and eggs are a great source of protein. </span></li>
<li style="font-weight: 400;" aria-level="1"><span style="font-weight: 400;">Switch to wholegrains for your rice, pasta or bread to follow the Mediterranean diet. </span></li>
<li style="font-weight: 400;" aria-level="1"><span style="font-weight: 400;">Have a healthy source of fat in your diet, from using extra virgin olive oil in your cooking to avocados, nuts/seeds, tahini or milk (within reason). It’s important to use </span><i><span style="font-weight: 400;">extra virgin</span></i><span style="font-weight: 400;"> olive oil, as not all olive oils are considered high in antioxidant phytochemicals. </span></li>
<li style="font-weight: 400;" aria-level="1"><span style="font-weight: 400;">Get creative and use fresh herbs such as basil or rosemary, garlic or spices to keep your dishes tasting delicious. Don’t forget an important part of the Mediterranean lifestyle is sharing food and enjoying dishes together, embrace this. </span></li>
<li style="font-weight: 400;" aria-level="1"><span style="font-weight: 400;">Drink plenty of water, and maybe even enjoy a small glass of red wine with your meal! But remember, as a bariatric patient we don’t really want you to drink while you are eating, so this should be an occasional treat. </span></li>

<li style="font-weight: 400;" aria-level="1"><span style="font-weight: 400;">Overindulge; just because something is ‘healthy’ doesn’t mean it isn’t high in calories or should be consumed in high quantities, a good example of this is cheese. </span></li>
<li style="font-weight: 400;" aria-level="1"><span style="font-weight: 400;">Consume highly processed foods. This is a very simple rule to follow, that will keep you on the right track. This will also help limit trans fats that are found in highly processed foods or fried foods (such as fast food or convenience meals). </span></li>
<li style="font-weight: 400;" aria-level="1"><span style="font-weight: 400;">Eat foods that are high in sugar like ice cream or fizzy drinks, or baked goods. Same goes for refined grains such as white bread or pasta. Avoiding both sugar and refined grains is an important part of the Mediterranean diet that will leave you feeling more energised throughout the day. </span></li>
<li style="font-weight: 400;" aria-level="1"><span style="font-weight: 400;">Drink your calories. It is very easy to drink excess calories through sweetened juice drinks, various fizzy drinks, alcoholic beverages or coffee. Instead focus on drinking plenty of water, natural fruit juices with no added sugar, and be conscious of how you are consuming your tea or coffee (use a natural sweetener instead such as Stevia or Monk fruit).</span></li>
